MTHFR is probably the most talked-about gene in nutrition, and the most misunderstood. So let's keep it plain: it's an enzyme that turns the folate you eat into the folate your cells can use.
The full name is methylenetetrahydrofolate reductase. Say it once, then forget it. What matters is the job. Folate from food and folic acid from a pill both have to be converted into one active form, 5-MTHF, before your body can do anything with it. MTHFR runs the final step of that conversion.
Some people's MTHFR runs at full speed. Some people's runs slow. The difference is written in two common variants of the gene.
The two variants worth knowing
The one most studied is called C677T. If you inherited it from both parents, your MTHFR enzyme works at roughly 30% of normal speed. From one parent, roughly 65%. The second variant, A1298C, has a milder effect on its own but can stack with the first.
These are common. Depending on ancestry, somewhere around 30 to 50% of people carry at least one copy of C677T, and roughly one in ten carries two. This isn't a rare disease. It's ordinary human variation, and it sat invisible for most of us until genetic testing made it cheap to see.
A slow MTHFR isn't a diagnosis. It's a setting. It changes which form of folate works for you, not whether you're healthy.
Why the form on the label matters
Here's the practical part. Most cheap B-complex supplements and fortified foods use folic acid, the synthetic form. Folic acid is stable and cheap, which is why it's everywhere. But it's inactive. Your body still has to convert it, and the last step of that conversion runs through MTHFR.
If your MTHFR is slow, some of that folic acid never gets converted. It circulates unused. You're taking the vitamin, the label says you're covered, and a chunk of it isn't reaching the pathways that need it.
The alternative is methylfolate, sold as 5-MTHF or L-methylfolate. It's the already-converted form. It skips the MTHFR step entirely, so a slow enzyme doesn't hold it back. For someone carrying the C677T variant, that switch is the single most useful change they can make to a folate supplement.
What folate is actually for
Folate isn't a standalone vitamin doing one thing. It sits at the centre of a process called methylation, which your body uses to build DNA, make and recycle certain neurotransmitters, and clear a compound called homocysteine out of your blood.
That last one is the clearest marker. When folate can't do its job, homocysteine tends to rise. It's one of the reasons a blood test for homocysteine is a useful companion to knowing your MTHFR status.
Folate also doesn't work alone. It runs alongside vitamin B12 and vitamin B6, and the MTHFR enzyme itself depends on riboflavin, vitamin B2, as a cofactor. Give a slow MTHFR enough riboflavin and it runs more steadily. This is why a good folate strategy is really a B-vitamin strategy, matched to how your enzymes actually work.
Where the science is solid, and where it isn't
I want to be honest about the edges. The biochemistry above is well established. That MTHFR variants slow the enzyme, raise homocysteine when folate or riboflavin is low, and respond to methylfolate: solid, decades of research.
The internet claims that go further, tying MTHFR to a long list of conditions, run ahead of the evidence. Some links are real and modest, some are contested, some are noise. If you read that a slow MTHFR explains everything about how you feel, be skeptical. It explains one specific piece of your nutrition, and it does that well.
