What the gene does
APOA1 encodes apolipoprotein A1, the main structural protein of HDL particles. HDL is the vehicle for reverse cholesterol transport, moving cholesterol from tissue back toward the liver. The variant sits in the promoter, so it changes how much of the protein gets made rather than altering the protein itself.
What the variant changes
In the Framingham study, women carrying the A allele had around 13% higher HDL cholesterol on a high polyunsaturated fat intake. Carriers of the common GG genotype, roughly two thirds of people, saw little to no HDL benefit from the same intake. In some carriers the direction reverses and omega-3 lowers HDL rather than raising it.
This is the kind of gene-diet interaction that gets lost in population averages. Pool everyone together and omega-3 looks mildly helpful for HDL. Split by genotype and you find one group driving the effect and another getting nothing.
What it means for you
Omega-3 does plenty of other things, and none of this argues against it generally. What it changes is whether omega-3 is the right tool if HDL is the thing you're trying to move.
Where it isn't, plant sterols and stanols are the better lever. At around 2 g a day they lower LDL cholesterol by roughly 8 to 10%, which is a well-replicated finding, and they do it without touching HDL in the wrong direction.
